Completion of 2 or more years of MIAA-level sport at BMHS (club sports are not applicable)
Family Membership in the BMHS Athletic Association in good standing (Dues paid for each year you played a sport)
Upload a one paragraph essay which demonstrates your leadership qualities
Upload one letter of recommendation from a BMHS teacher
Upload one letter of recommendation from a BMHS coach
Material must be received by BMHSAA on or before Friday, April 3, 2023
